Housing Outreach and Treatment (HOT) Team members can use this portal to access the most up to date documents, referral forms, and information necessary for street outreach.

Log In to an Existing Account
- If you have not established an account, please request one by completing the Online Request Form). If approved, you will receive an e-mail with your initial login information.
- Once logged in, you will have access to private areas containing documents. Simply click on the "Downloads" link and select the document you are looking for
